Information Needed to Set Up Your Payroll

Required Information Checklist


  • Proof of quarterly federal and state tax deposits for each prior quarter of the current year

  • Proof of federal and state tax deposits for each payroll of the current quarter

  • Unemployment rate - the letter you receive with your rate is a plus!

  • Login access to your current payroll system so we can pull the payroll info

    • If you are sending payroll info to us, we need all prior payroll info for the current year:

      • Demographic info for ALL employees who have been on this year’s payroll (a spreadsheet template is attached)

      • Quarter-end reports for each full quarter completed this year

      • Partial quarters can be given as whole months or individual pay periods

      • Should contain complete breakdown per employee of all types of pay and deductions as well as employee and employer taxes

      • Preferably in .csv format, however .xlms is good and .pdf is fine.  

    • Inside Dropbox under the Agreements → To be signed:

      • Electronic Signature Request Form

      • If you need them for employees, you can also find blank forms W4, i-9, and employee direct deposit authorization (we don’t need current employees to fill out new forms unless they want to update something)

    • From Zoho Sign (emailed to you)

      • SC 2848 (and other states if applicable)

      • UCE 1010

      • NatPay DDPlus form

    • From AccountantsWorld (emailed to you from the payroll system)

      • EFT application (Yodlee or penny test)

      • Form 8655
